THE IMPORTANCE OF PATIENT-BASED ACTIVE SURVEILLANCE IN NEUROSURGERY DEPARTMENT |

The purpose of this study was to determine the frequency and risk factors of nosocomial infections in patients followed in a
department of neurosurgery. The patients hospitalized in the Clinic of Neurosurgery, Ondokuz Mayıs University Hospital, between
May 1 and September 30, 2007, were prospectively investigated. At the end of 5-month period patient-based surveillance we
detected nosocomial infection in 142 (22.8%) of 622 patients. Significant risk factors according to logistic regression analysis were
extended (long) hospital stay, H2 receptor blocker use, total parenteral nutrition, urinary catheterization, external (surgical) drains
and general anesthesia. The most commonly encountered infection was meningitis (52/142; 36.6%). Cerebrospinal fluid (CSF)
cultures were negative in 29 of these 52 patients (55.9%), and the diagnosis was based on clinical findings and microscopic
examination of CSF. Gram negative microorganisms were the most commonly isolated bacteria. The mortality rate was 15.6% in
general and 33.8% in patents with nosocomial infection. The results of this study show the importance of patient- based surveillance
in the early diagnosis of culture-negative nosocomial infections in units with high nosocomial infection rates.
