THE FIRST PARENTERAL STREPTOGRAMIN: QUINUPRISTIN / DALFOPRISTIN

Combination of quinupristin/dalfopristin, not in use yet in Turkey, is the first antibiotic of streptogramin A group that is parenterally applied. Quinupristin, a derivative of pristinamycin IA, is from the streptogramin A group. Dalfopristin, derived from pristinamycin IIB, is an antibiotic of the streptogramin B group. The mechanism of action of both of these two components is by binding to 50S subunit of bacterial ribosome. This combination applied by intravenous infusion includes 30% quinupristin and 70% dalfopristin. In spite of the necessity of its application by catheter and side effects such as artralgia and mialgia, quinupristin/dalfopristin is an effective choice of antibiotic in the treatment of severe and recalcitrant infections caused by Gram-positive pathogens revealing multiple drug resistance, such as Staphylococcus aureus and Enterococcus faecium.
