EXTRA-PULMONARY TUBERCULOSIS IN ADULTS: AN ANALYSIS OF 67 CASES

Tuberculosis is a disease that can involve every organ system. In this study, a total of 67 adult patients of extrapulmonary tuberculosis diagnosed and followed between 1996 and 2003 were retrospectively evaluated. Of patients, 36 (% 53.7) were female. The mean age was 35.8 (range, 14-85). Underlying medical problems and predisposing factors were present in 10 (% 14.9) patients. Twenty (% 29.8) patients had prior history of tuberculosis. Presenting symptoms were protean, and often resulted in long delays between onset of symptoms and eventual diagnosis. Therefore, the duration lasting before diagnosis was longer than one month in % 53.7 of patients. There were identified anemia in 35 (% 52.2), leucocytosis in 20 (% 29.8), increased ESR in 37 (% 55.2) patients. Chest radiograms were normal in 49 (% 73.2) and tuberculin skin test was positive in 33 (% 49.2) cases. Cultures showed growth of Mycobacterium tuberculosis in 16 (% 26.6) of 60 affected sites. Caseating or noncaseating granulomas were identified in 19 (% 82.2) of 23 cases. Central nervous system tuberculosis (% 53.7) was the most common extrapulmonary tuberculosis type and was followed by bone and joint tuberculosis (% 20.9), lymph node tuberculosis (% 9), miliary tuberculosis (% 6), periton tuberculosis (% 4.5), cutaneous tuberculosis (% 4.5) and pleural tuberculosis (% 1.4). Extrapulmonary tuberculosis is not an uncommon disease. A high index of suspicion is required in making the diagnosis of extrapulmonary tuberculosis.
