Cryptococcal meningitis generally occurs in immunosuppressed patients, organ transplant receivers, in patients with malignancies and in acquired immune deficiency syndrome (AIDS). Cryptococcal infection is rare in the immunocompetent. This paper describes a case of cryptococcal meningitis in a previously healthy 72-year-old man. The diagnosis was based on clinical findings, the results of cerebrospinal fluid (CSF) analysis, and magnetic resonance imaging (MRI) findings. Cranial MRI revealed enlargement in the ventricles and increasing hydrocephalus. A lumbar CSF draining set was installed in order to lower the ventricular pressure. The patient was successfully treated with amphotericin B, flucytosine and fluconazole.
