Group C streptococci are common pathogens in animals, however, they cause rare but serious infections in humans. A 82-year old male case was admitted with one day history of high fever, weakness, altered consciousness and urine incontinancy. Coronary arterial disease, hypertension and diabetes mellitus were defined in his history. On physical examination the findings were as follows: temperature, 39.2ºC; blood pressure, 170/90 mmHg; pulse rate, 110/minute; respiration rate, 28/minute. Alteration of consciousness and neck stiffness were found; Kernig's and Brudzinski's signs were absent. Laboratory investigations were as follows: WBC count, 37220/mm³; platelet count, 12100; erythrocyte sedimentation rate, 66 mm/h; blood glucose, 208 mg/dL; urea, 56 mg/dL; creatinin, 1.5 mg/dL, and the other biochemical tests results were normal. In cerebrospinal fluid (CSF) examination, leucocyte count was 4000/mm³, protein was 300 mg/dL, and glucose was 34 mg/dL (simultaneous blood glucose level was 208 mg/dl) and CSF pressure was high. Ceftriaxone 2x2 gr (IV), anti-edematous and supportive therapy were started. Group C Streptococcus was isolated from the CSF cultures and identified as Streptococcus zooepidemicus. Gentamicin 2x80 mg/day and rifampicin 600 mg/day were added to the+ treatment. The patient was lost on the third day of his admission. The case is discussed due to the rare appearance of acute group C Streptococcus meningitis in the literature and high mortality rate of meningitis with this group of pathogens.
