THE EVALUATION OF CASES OF TUBERCULOUS MENINGITIS

Thirty-three adult cases of tuberculous meningitis hospitalized in the Clinic of Infectious Diseases, Izmir Teaching Hospital, İzmir between 1996-2002 were evaluated with respect to history, clinical and laboratory findings, treatment and prognosis. The mean age of the patients was 39±16 years with a range of 16 to 70. Almost all the patients had fever, headache, nausea and vomiting. On physical examination meningial irritation findings (100%), alteration of consciousness (82%), diplopi (24%), strabismus (12%), facial paralysis and hemiparesis (9%) were observed. Thirty percent of the cases were diagnosided to have active pulmonary tuberculosis. The confirmation of the diagnosis was made through cererospinal fluid (CSF) examination and clinical data. Acid-fast bacilli were isolated from CSF in 18%. Nine (27%) cases died; all were in the third stage of the disease and had underlying diseases. One (3%) patient developed hemiparesis as sequela. In conclusion; tuberculous meningitis is associated with a high mortality and morbidity. For this reason early diagnosis and effective treatment are important for favorable prognosis and preventing sequelae.
